Community Living Connections is both a call center and an an extensive, reliable service provider network supported by HSD's Aging and Disability Services division. It is the "front door" to many other services we fund and/or coordinate to promote healthy aging, including:

• Care coordination
• Caregiver support
• Case management (in-depth assistance and personal care that helps older adults and adults with chronic health needs continue to live in their own homes)
• Kinship care
• Minor home repair
• Nutrition (community and home-delivered meals and the Senior Farmers Market Nutrition Program in Seattle-King County)
• Senior centers
• SHA case management (connecting older adults and adults with disabilities in 52 SHA buildings to services that help avert crises and prevent eviction)
• Volunteer transportation
