Street Maintenance Request Form Thank you for visiting SDOT's Street Maintenance Pot Hole reporting page. Our goal is to repair potholes within three business days of receiving a report. Use this form to report non-emergency problems such as pot holes and other minor street repairs, sidewalk safety concerns, malfunctioning traffic signals, and damaged street signs. If you have a concern about vegetation overgrowth, please use this link to get to the appropriate complaint form. For other complaints (such as graffiti, noise, air quality, etc.), please see listing of other complaint services. Please Note: The City of Seattle has implemented a new application to support customer service requests from constituents. The new application provides a more efficient way to submit and track requests for service inquiries and/or complaints submitted to the City.
